We are looking for someone who has a positive approach, is self motivated but works effectively in a team. If so this may be the job for you. The successful candidate will have previous experience of key working young people in a similar vulnerable client group, ideally within a lone working environment. Experience of assessment and support planning systems and electronic case-management systems for this client group is essential. They will have the ability to communicate with and motivate reluctant and hard to engage young people to enable them to achieve positive change in their lives. A knowledge of resettlement issues and relevant legislation, along with an understanding of Child Protection is essential. The successful candidate will work intensively with young people with high support needs to maximise their abilities and opportunities to resettle and maintain their own tenancies. They will draw up with young people with high support needs, detailed and tailored Support Plans, to assist them through the process of building stability and independence in their lives. They will motivate and assist young people to undertake the achievable steps outlined and agreed with them as part of the Support Plan. They will liaise with and facilitate young people’s involvement with relevant external specialist agencies, which may assist young people in the achievement of their goals as identified in their Support Plans.
