Dr. Glenn shows how he is using the https://cad-ray/ system designed by the renowned Dr. Armen Mirzayan DDS. Introduction: I decided to give Arman’s service a try and see how it went. I was secretly hoping hed send me a stent that caused me to perf the bone so I could make a thread entitled something like Armen sucks and joins the perforator club! : )) ................but, I must say I was pleasantly surprised at the ease of the process and the procedure. I will be using his services again. Now I know Ive been a big proponent of DIY guides when coupled with CT info and I still am when theres plenty of bone. However, Im hoping to at least double the number of implants I place this year and in order to do that, Ive got to streamline my processes and farm out some of the lab work even though I really enjoy it. This process is very streamlined and requires minimal work on my part. I upload my CT through their web portal, send a stone model, and fill out the prescription online and thats it............stent comes back along with a recipe for placement. Heres my patient........I just edentulated his left side so now hes motivated to get the right side done. My cone beam showed there was adequate bone but there were some strange angulations and undercuts and placing these parallel was going to be near impossible without a guide so I though good first case to try it on. The following are all the images emailed to me for the plan so that I could OK them before fabricating the stent. Absolute parallelism wasnt possible due to angles of the mandible but its close enough that I can use stock abutments to restore. Buccal is sutured to the cheek and lingual flap has a long stitch hanging out with the needle driver clamped on it and just hanging off the opposite side of the mouth Now the stent fit perfectly but keep in mind that it was designed on the tissue and there are no posterior solid stops which means it would rotate downward as I pressed on it with my drills. So I used liquid triad to create a bony stop after we flapped. I placed the triad between the implants and then placed a layer of sterile Teflon tape over it and seated in back in the mouth so that the triad squished against the bone. My assistant cured it and no we have a solid stent with a posterior stop against bone and no movement Osteotomies took about 2 minutes to make Blue Sky Bio 3.5 x 10 at #28 Blue Sky Bio 4.3 x 8 at #30 Closed up Conclusion: Ill put some pics up later of my guided drills but Armen convinced me to use the Camlog guided drills and I liked them a lot. They only come in 3.8 and 4.3 diameters but the though is that these are the most common sizes......if youre going smaller than that then just get a pilot hole stent. If youre going bigger, then no big deal, youll probably have to use your final drill from your kit free hand to finish the osteotomy. I liked them a lot and they had very little wiggle room. I think I paid around $200 for each set of drills so $400 total. They include 5 drills........all are the final diameter, you just increase progressively in length. Again, very happy with the case. I could have done it free hand but this was very easy and we were done with the whole procedure in an hour even with numbing, xrays, hygiene checks, and pictures so if it makes me more productive, I dont mind dropping $200 for a stent.
