As such, the evaluation (including the consult and ultrasound) should be covered by most, if not all, insurance companies as long as signs and/or symptoms of venous insufficiency are present and are documented properly. Ultrasound evaluation must document significant insufficiency in one of the named saphenous veins. After a trial of conservative therapy with exercise, compression hose and elevation have been completed, a request for authorization of endovenous ablation may be submitted and in most cases the procedure will be authorized.
