We had a great time at #TNP2014 this past weekend! Heres your Monday Challenger: A 38-year -old man presents to the Nurse Practitioner with symptoms of headache, sore throat, and temperature of 101. He states the headache began about 3 days ago with a sore throat, but the fever began yesterday along with generalized muscle aches and general malaise. This morning he experienced 3 episodes of diarrhea. He denies nausea but states he has occasional stomach cramps and noticed a faint rash on his arms, but denies itching. Past medical history non-contributory. His takes a vitamin supplement, exercises at the gym routinely and is mindful of a healthy diet. He is a project manager for a major transportation company and travels all over the continent. He also belongs to a missionary group and travels 3-6 weeks a year to underdeveloped countries. Examination: Patient appears somewhat pale and tired; Weight 175 pounds and height 69 inches; B/P 142/76, Heart rate 88, respirations 22, temperature 101.6 °F. Eyes – bilateral conjunctival injection; erythematous throat without exudate; nasal turbinates swollen and pale without discharge; bilateral cervical glands 2 cm and tender; Heart – RRR; Lungs clear bilaterally; erythematous maculopapular rash noted on both arms and upper chest with some desquamation noted on the arms; abdomen soft, mild hepatomegaly noted; remainder of exam WNL. Tests: Stool: Occult blood positive CBC: Leukopenia; Decreased H/H; Platelet count decreased Sedimentation rate elevated Amylase elevated Hepatic transaminases elevated Urine: Proteinuria Prothrobin (PT) and Partial Thromboplastin times (PTT) prolonged Based on the history, findings, and lab results, what should be included in the differential diagnoses?
