ACCICARE DISCOUNT CLUB Explained. AcciCare Discount Club was established, sometime in 2011, for the following reasons: 1) To provide an affordable, a flexible, and a comprehensive microinsurance platform for everyone, 2) To provide a venue for entrepreneurial opportunity amongst the club members by harnessing the cumulative buying power of the club membership, and 3) To provide a stable, consistent, long-term, equitable, and a renewable income resource for each and every member. It started small, as an experiment, to test its viability. It was initially offered – for FREE – as a discount card privilege card to a few selected Group Student Accident Insurance client-schools in Cagayan de Oro City. On its 2nd year, we started offering our initial paid complete membership package called AcciCare Basic Plus, consisting of - (1) an Annual Renewable Accident Insurance plan of P50,000; (2) a 1-Year Discount Card, and (3) an Affiliate Referral Incentive program for members to earn a modest P50 for each individual referral made – to groups and individual clients. All this for a modest annual club membership fee of just P300 per member. It grew step-by-tiny step; tweeked and fine-tuned at every stage. To date, there are already about 2,500 card carrying members of AcciCare Basic in Cagayan de Oro and Iligan Cities – both groups and individuals, private and public employees, persons from 18 years up to 65 years, from all strata of life. That is in addition to 20,000+-strong card-carrying FREE grade school/high school & university student-members in Cag de Oro schools & a prestigious university. Now, on its 4th year, we envisioned to reach-out unto a bigger arena, not only in Cagayan de Oro but beyond – by way of the traditional person-to-person referrals, as well as by the use of social media and the internet. Now is the time for AcciCare Elite Plus. 1. WHAT IS OUR PRODUCT? AcciCare Elite Plus Discount Club is an annual renewable club membership. As a Club Member, you would : 1) Be Insured. Each member is covered by an Annual Renewable Comprehensive Accident Insurance. 2) Earn Discounts. Each member is entitled to discounts from our Accredited Club Business Partner-Establishments on goods purchased or services rendered. 3) Harness the memberships buying potential. If you are a business owner or a service provider, you automatically become a Club Business Partner-Establishment in exchange of offering your own individual discount schemes to each buying member that comes into & patronizes your store or business 4) Earn Affiliate Referral Incentives. Yes, you earn for each successful club membership you refer. And EARN BIG, infinitum. All of the above are Annual Renewable – meaning you get to be Insured, Earn Discounts, Harness the clubs buying power for your business, and Earn Incentives – Year-in-and-Year-out, agan and again, EVERY YEAR, each time you and your Referrals renew your club membership. Potentially for Infinity. For your entire lifetime. 2. INSURANCE PACKAGE & ITS BENEFITS? Eligibility Any Filipino who: • Is residing in the Philippines • Within the age of 18 years minimum, up to a maximum age of 60 years • Is NOT DIAGNOSED TO HAVE A DREADED DISEASE (like but is not limited to Cancer, HIV, or any serious life-threatening disease) at any time PRIOR to the membership application. As an insurance-qualified Club Member, you are entitled to the following insurance benefits: Accidental Death & Disability 100,000 Unprovoked Murder & Assault 50,000 Accident Medical Reimbursement (per accident) 10,000 Burial Assistance Benefit – Accidental Death 10,000 Burial Assistance Benefit – Natural Death 10,000 Mortuary Memorial Service Benefit – All Causes of Death 20,000 Cash Assistance Benefit – All Causes of Death 20,000 Daily Hospital Income – Accident Confinement, 15 Days/Yr. 150/day Fire Assistance Benefit 1,000 Ambulance Assistance Benefit 1,000 IMPORTANT NOTES: a) Insurance Term is for One Year. b) Insurable Eligibility Age is from 18 years up to maximum of 60 years old only. c) All members, for insurance purposes, are subject to the 1 year contestability period; Illiterate members shall have a 2-year contestability period. The insurer reserves the right to contest any claim happening only within the 1st year. d) Insurance membership benefits are transferable. A member may transfer the whole insurance cover to another person, upon his written request, and upon satisfaction of other transfer requirements as stated in the insurance policy. e) Motorcycling-related accidents are covered. f) All members shall have their own club membership discount cards/id card. g) Insurance policy copies may be forwarded to the members, via email or regular mail, upon written request. INSUREANCE BENEFITS EXPLAINED The insurance program is an ALL RISK ACCIDENT COVER that provides 24/7 protection to Club Members with ages between 18 years up to 60 years of age, for One (1) Whole Year, anywhere in the world, whether at work, or at home. The Insurance Program also covers the Club Member while they are on travel in a private or public conveyance using land or sea transportation, or while travelling as a fare paying passenger on any commercial, scheduled or non-scheduled, special or chartered flights. Motorcycle-related accidents, Drowning, animal/dog bites, snakebites are also covered. Sample Illustations: Scenario 1 – The insured Club Member was onboard his motorcycle when he was involved in a road accident, was confined in the hospital for 15 days, incurred a hospital bill of P100,000, and ultimately met his untimely death due to the road accident. His applicable insurance claims benefit proceeds would consist of the following: • Hospital Income Benefit 15 Days x P150/day = P 2,250.00 • Accident Medical Reimbursement (maximum) = 10,000.00 • Cash Assistance Benefit = 20,000.00 • Burial Assistance Benefit = 10,000.00 • Memorial Service Benefit = 20,000.00 • Accident Death Benefit = 100,000.00 TOTAL BENEFIT RECEIVED = P162,250.00 Scenario 2 – The insured Club-Member was confined in a hospital for 8 days due to Dengue Fever: His applicable insurance claim benefit proceeds would consist of the following: • Hospital Income Benefit, 8 days x P150 per Day = P 1,200.00 If he succumbs to the Dengue Fever and expires: His applicable insurance claim benefit proceeds would consist of the following: • Hospital Income Benefit, 8 days x P150 per Day = P 1,200.00 • Cash Assistance Benefit = 20,000.00 • Burial Assistance Benefit, Natural Death = 10,000.00 • Memorial Service Benefit = 20,000.00 TOTAL BENEFIT RECEIVED = P 51,200.00 Scenario 3 – The insured Club-Member was fatally injured when he was tapped on the head with a hammer while he was sleeping at home by an akyat-bahay burglar. His applicable insurance claim benefit proceeds would consist of the following: • Murder & Assault Benefit = 50,000.00 • Cash Assistance Benefit = 20,000.00 • Burial Assistance Benefit = 10,000.00 • Memorial Service Benefit = 20,000.00 TOTAL BENEFIT RECEIVED = P100,000.00 INSURANCE BENEFITS, DEFINED Accidental Death and Disablement Benefit (Php 100,000.00) When an Accident Injury results in the untimely Death &/or Disablement of the insured Club-Member within 180 days after the date of accident, the company shall pay the benefits subject to the percentage of the principal sum provided in the policy schedule AD&D Principal Sum – Php 100,000,00 Unprovoked Murder & Assault (Php 50,000.00) Should the Insured-Member met his untimely death as a result of unprovoked murder or assault, their Beneficiary is entitled to the sum of Php 50,000. Accident Medical Reimbursement (Php 10,000.00) When if by the reason of ACCIDENTAL INJURY, the Insured Club-Member shall require • treatment by a legally qualified physician or surgeon • confinement in a hospital • the employment of licensed or graduate nurse the company will REIMBURSE the actual cost of all necessary medical treatment, hospital care, nursing services, medicines, x-rays and all other reasonable medical expenses incurred within 52 weeks from the date of the accident, up to a maximum amount of the maximum amount of Php10,000.00 Burial Assistance Benefit – Accidental &/or Natural Death (Php10,000.00) The Insured Person/Club Member is entitled to a Burial Assistance Benefit in the amount of Php10,000, if death should occur within the insurance policy term as a result of Accidental Bodily Injury or as a result of Illness and Natural Causes. Mortuary Memorial Service Benefit (Php 20,000.00) The Insured Club-Member is entitled to the full and complete scope of memorial services, up to the Maximum Amount of P20,000, by an Eternal Plans Inc.-accredited mortuary nationwide upon death due to an accident or illness. This benefit is NOT ASSIGNABLE & NOT TRANSFERABLE, meaning it cannot be used by any deceased person other than the insured Club-Member. The Scope of Memorial Services that the Insured Club-Member is entitled to includes the following: • Phase 1 – First Call In the event of death of the Insured Club-Member, the family has to relay the message to any one of the branch offices of Eternal Plans located nationwide. The Memorial Service Administrator (MSA) of Eternal Plans then coordinates with the accredited mortuary to initiate the provision of memorial services. They may also contact Eternal Plans – 24 hours, 7 days a week – at the number attached to the members Contract/Life Plan Card issued to each insured Club-Member. • Phase 2 – Preservation, Cosmetics, and Casketing The Servicing Mortuary shall facilitate the initial servicers : preservation of the body (embalming), application of suitable cosmetics and placement in an appropriate chosen casket. • Phase 3 – Viewing and Final Arrangements The Servicing Mortuary may provide facilities for wake for a maximum of 5 days. • Phase 4 – Trip to the interment site The Servicing Mortuary transports the body to the interment site. The plan benefit covers 25-kilometer radius. ETERNAL LIFEPLAN CARD Instead of the Policy Contract, each insured Club-Member shall be issued a “LIFE PLAN CARD” together with a copy of the general provisions of the Life Plan. The Life Plan Card is the Club-Members identification card. It contains the important personal details of the planholder, the plan benefits, and the terms and conditions of the use of the card. Re-issuance of a lost Life Plan Card will require payment of applicable fees. Other Matters Other mortuary services rendered not mentioned or covered herein is for the account of the Club-Member’s beneficiary or family. If the memorial service benefit is unrendered, Eternal Plans shall pay to the Club-Member beneficiary Seventy (70%) Per Cent of the Live Plan Value, or Php14,000, as un-rendered service benefit. Insured Club-Members shall be subject to the 1-year Contestability Period; Illiterate members shall have a 2-years Contestability Period. The insurance company reserves the right to contest any & all doubtful claims happening within the 1st year. Cash Assistance Benefit – All Causes of Death (Php 20,000.00) Should the insured Club-Member die during the coverage period, their Beneficiary shall be given an additional Cash Assistance Benefit in the amount of P20,000 by Eternal Plans Inc. This is to address their immediate and urgent needs of the family beneficiary. Daily Hospital Income – Confinement due to ACCIDENTS &/OR ILLNESS The Club Member is entitled to a Daily Hospitalization Income, in the amount of Php150 for each day of hospital confinement due to an accident and/or illness, starting on the 1st day of hospital admission and up to a maximum payable period of 15 days within the policy year. Ambulance Assistance Benefit (Php1,000.00) The insured Club-Member is entitled to reimburse fix amount of Php1,000 per year as financial assistance whenever they avail of an ambulance service due to an accident or illness. Fire Assistance Benefit (Php 1,000.00) The insured Club-Member is entitled to reimburse fix amount of Php1,000 per year as financial assistance whenever their residential house is burned down by fire. 3) ACCICARE ELITE PLUS DISCOUNT CARD All AcciCare Elite Plus Discount Club members shall be issued their individual AcciCare Elite Discount Cards each time a club member renews his club membership on an annual basis. The Accicare Elite Plus Discount Card is the Club-Members club identification card. It contains the important personal details of the planholder, his insurance plan benefits, and the terms and conditions of the use of the card. The card entitles the club cardholder-member to SPECIAL DISCOUNT privilege packages from accredited partner-merchants and establishments nationwide. Club Member-Cardholder can gain access to value-added services such as special discounts in Hotels & Resorts, Shopping Centers & Retail Stores, School & Office Supply Stores, Bars & Restaurants, Spas, Health and Wellness stores, and more. The list of Accredited Partner-Establishments would be posted on the Club’s Facebook Web Page for easy reference by the club members. It would be updated periodically. The Card also entitles the Club-Members some PRIVILEGE SPECIAL INSURANCE RATES each time they avail of the insurance services of Fortune General – Cagayan de Oro Branch.
