ANNUITY & SAVINGS FUND The letters and applications for the In-Service Distributions (2 year money) have been mailed to all participants who have funds available to withdraw on June 1st this year. If you wish to withdraw these funds, you need to complete the Local 132 cover letter along with the United Bank In-Service Withdrawal Request form and return to the Trust Office. A summary of how to complete the form is below: Section A - enter your name and social security number, date of birth and phone number; Section B - is already marked that you are employed with the Union and have contributions with the Fund; Section C - chose how you want your money paid (to you or an IRA rollover), enter either 50% or 100% and the Contribution Source is ERB5; Section D - only used for a rollover/annuity with another Provider; Section E - how you want to receive your payment (check, express delivery or direct deposit); Section F - only complete if you want additional Federal taxes or state taxes withheld, the Fund already is required to withhold 20% Federal taxes from your distribution unless you are doing an IRA Rollover; Section G - Participants signature, you only need to have a Notary witness your signature if you are changing your mailing address; Section G - Spousal consent, if you are married your wifes signature is required, witnessed by a Notary; If you have any questions about the forms, you can call the Trust Office at (304) 525-0482 or 1-800-642-3525. Mail the cover letter and application to the Trust Office (address shown at top of our letter) and we will complete the Plan Administrator section and forward to the Plan Service Center for processing. They are to begin issuing payments on June 2nd.
