Answer to Thursdays ACLS Certification Institute Question of the Day: The defibrillator delivers a shock synchronized by sensing the R wave of the ECG in order to avoid the vulnerable phase of the cardiac cycle and to decrease the risk of initiating ventricular fibrillation. The period of vulnerability is near the apex of the T wave, which represents the terminal portion of the refractory period. At that time, adjacent fibers in the heart are in various stages of repolarization. Cheers! Dr. S.
