Anyone who has lost their children from crown wards to adopted.. make sure you get the Social History Questionnaire form. It goes in the childs file.. you fill it out, can do it over the phone with a worker and then have a faca to face meeting about it. You are allowed to put in it the best way for your children to contact you…should they wish to do so. They also get to read about their parents from parent(s) point of view. CAS did give me a hard time and yes I had to go to the CFSRB …grandparents and other family members are also allowed to do their own Social History Form to be left for the child(ren).
