August 19, 2013 TO: Withlacoochee Electric Cooperative, Inc. ATTN: CUSTOMER SERVICE FROM: Neal E. Wilborn, Jr. RE: Member ID# 0051864003 – Electronic Transfer Authorization Form To Whom It May Concern, I am enclosing a copy of my signed and dated electronic funds transfer authorization form, along with a copy of the original voided check I mailed on 7/3/13. Apparently, the original I mailed did not reach its intended destination. As a result, I am mailing this form to each and every one of your branch offices. While we are on the subject of customer service, I would like it noted that the service I have received has been atrocious. My father passed away this past September, and I inherited the property that Withlacoochee services. I have had nothing but issues with your company. I have been misinformed of payment amounts and required account information, been told that things were handled, when in fact it they were not, accounts shut off without notice, several unnecessary trips made to Crystal River from Gainesville by myself and my office manager, and the most recent: a mailed copy of my authorization of electronic funds transfer form that has somehow disappeared in the mail. I couldn’t even begin to describe the frustration I have experienced with your customer service team at the Crystal River location. I can only hope at least one of the five copies I’ve sent out will reach its destination, and then I won’t have to experience your failed attempt at customer service. Regards, Neal E Wilborn, Jr.
