Dear Health Care Professionals/Volunteers, It is once again time for DOCARE/Midwestern University to start preparing for our upcoming medical outreach 2014. We will be headed back to Guatemala. The mission will involve aiding communities we have visited on prior missions. During the first week we will work in and around San Andres, Itzapa and then spend most of the second in Tecpan. The dates for this year’s mission are February 15th to March 1st . Medical healthcare professionals/volunteers may choose to stay one or both weeks and we ask that you clearly designate what your preference is on your application (first week, second week or both). Since this is an official DOCARE Mission, we would like all those who join us to be members of the organization. Thus there are two forms to complete: 1. Mission application (this needs to be completed now please follow the steps listed below). Once I have received this application and have ensured we still have availability on our team I will send you the Guatemalan Medical Mission informational packet and ask that you then please sign up for DOCARE membership 2. DOCARE membership application -Please wait until you are formally accepted to attend the mission before filling out the DOCARE membership application Mission application: docareintl.org/ 1. Click to enter site. 2. Click on “Upcoming Outreach Projects” 3. Click on the mission that reads: GUATEMALA San Andres February 15 - March 1, 2014 There are many missions to Guatemala around this time so please make sure you register for the “San Andres” correct mission (Drs. Burdick and Schalscha will be the operational officers listed). 1. Look for your specialty and click on the “VOLUNTEER TAB” Click the category that best fits your skill set, though this is NOT crucial and if your specialty is not present enter your information under the general “Volunteer” heading. The DOCARE website will automatically forward these applications forms to me. Due to the large group of volunteers and limited availability we ask that you please submit you application as soon as possible. We will fill our roster on a first come first served basis. If you have any questions please feel free to contact me. As stated above I will send out a acceptance note and a mission packet once I receive your application. Please give me a week or so to get back to you as I try to group my responses. At a later date….once accepted formally to the mission you will need to return to this site to complete a membership application. Also, this year evacuation insurance is required by DOCARE in order to attend a mission. We will not be asking for your proof of coverage but ask that you attain it.
