Dear Sirs, I am writing to inform you about the accident which took place on 15 September 2014, on the premises of the „Alpla” plant at 69 Lesnianka street in Żywiec - Poland. The accident resulted in the death of Stanisław Adamczyk, my beloved husband and the father of my children, together with whom I am writing this letter. On the day of his death, my husband had been working in the warehouse for 21 years and was an employee with vast experience. Together with the present Management of Alpla Żywiec, he actively participated in the development of the Żywiec plant from its very beginning in 1993, performing different functions e.g. that of the warehouse manager. He was a friendly, outgoing person, much appreciated by his colleagues and co-workers at Alpla Żywiec. During his 21 years at Alpla, he proved his loalty and commitment to the company again and again by great devotion and involvement in performing everyday tasks. On the day of the fatal accident, my husband left home at 5:30 a.m. to begin work with the first shift. Little did we know that we would never see him again. As his colleagues say, he was very cheerful and full of energy that day and kept working till 8:16, when the tragedy happened. On getting acquainted with the post-accident report and with the evidence in the form of picture documentation and the CCTV recordings, we were shocked – and still are – by the terrible conditions in which my husband so faithfully performed his daily duties. The warehouse of „Alpla” , such a dynamically developing company, looked terrible. There were no lines that would indicate the place for storing pellets and other products. No transportation routes were marked and there was a lack of proper lighting. The Big-Bag type of products were stored very carelessly. There were no procedures on the ways to store the bags. The pallets used for storing the bags were very poor quality and were unsuitable for multiple use. Numerous bags of the Big-Bag type were damaged and the pellets were pouring out onto the floor. The bag repair carried out by the workers consisted in sealing the holes with an ordinary sticky tape. The post-accident documentation stipulates that my husband drove his fork-lift truck into an alley between two rows of pellet bags of the Big-Bag type, stacked up in two layers. While going in the reverse gear, he stopped the truck, got out of it and came up to one row of the bags with pellets. It was then that one of the bags, weighing 1200 kilograms, slipped down on him, killing him on the spot. The bag slipped as it had been put on top of another bag, which was torn and uneaven. The CCTV records show that my husband died at 8:16 but was not fund until 8:50! My husband’s friends were trying to reach him but here another obstacle was the fork-lift truck my husband had driven, as it would not start for no apparent reason. We have still been unable to comprehend how the „Alpla” company, who so highly values safety explicitly mentioned in its quality policy, can put people’s lives at risk every single day. We were stupefied to see in the photographic documentation the drastic conditions in the warehouse and the negligence on the part of the company management regarding them. What is especially shocking is the fact that despite the numerous identified negligence areas, in the post-accident report the Alpla Żywiec company does not list any irregularities or faults on its part. We regret to say that after such a tragic and shocking accident we have not received any words of sympathy, comfort or any help from the management of „Alpla” in the time so very difficult for us. As the family, we are going to create publicity around this tragic accident since – as we perceive it - until now the position of the management of Alpla has been totally unacceptable. We are under the impression that the tendency is to cover up the irregularities and abnormalities in the functioning of Alpla, Żywiec. The law office of Mr Rafał Bargiel, the barrister, will support us on this matter. Our aim is to appeal to other Alpla plants all over the word to address the problem of negligence concerning the worker’s safety, so that no family ever has to live through a tragedy as horrible as ours.
