We ask that when you request our services that you have the following immediately available so we are able to provide the highest level of care and make the transition smooth for the patient. When arranging for patient transport, have the following information available: Patient name Age, including date of birth Diagnosis, presenting problem, or mode of injury Vital signs Pertinent laboratory / diagnostic data (if available) Treatment received Contact phone number Prior to First Care Ohio Teams arrival, please make a copy of the medical records, X-rays, and pertinent laboratory data. First Care Ohio encourages early activation of our BLS and ALS services.
