Hey guys... I need some opinions/advice. My Luke is 3.5 months old and just recently had a spurt of not throwing up after one hot mess of a 108+ day current nicu admission. We have 3 groups following him; surgery, neonatology and GI. The GI doc today asked me how Id feel about home tpn (picc line, 24hrs) while advancing on feeds. He is up to 35 ccs every 3 hours by mouth, without issue (not one vomit.. No belly distention..poops 2-4 times per day. Almost 2 weeks back at eating after 2 weeks of npo from a case of medical NEC). Sidenote is he has a trach that is ready to come out but they were going to leave in just in case his NEC turned surgical. My question is, aside from personal needs/wants which I dont care about.. Would you guys advance at home or stay in the hospital, if given the option. Im looking for safety points.. Or experiences that could educate me on why I might not opt for the home route. Im worried about biting off more than I can chew but since he was born, I also noticed every change in him that led to his 3 work ups for sepsis + NEC (meaning I miss nothing symptomatic ). I want Luke home but more so want him safe. Any bits of advice to offer on the pros/cons of electing to do home tpn while advancing to full feeds (currently needs 93 ccs to be full feeds) Sorry about the novel and thank you in advance for your help!
