I felt sorry for the workers. It shouldnt happen. All accidents are preventable. Its common in any investigation, the causes of these accidents are due to the workers not following procedures, not using the proper equipment, not wearing the proper PPE. But the root causes are usually within the organsiations fault, i.e. the construction company and their client, government. They dont effectively manage, enforce or monitor the safety and welfare of the workers.
