I have been warning patients as well as clinicians for years that we should stop using these medications indiscriminately. Before the advent of Prozac and the subsequent plethora of new generation antidepressants we were extremely cautious to use the available tricyclics only in clear cases of endogenous depression. Avoiding them in reactive depression altogether. (Mild to moderate depression will generally meet the guidelines for reactive depression if the depression is also precipitated by a traumatic external event. Reactive depression differs from endogenous depression as reactive depression is not believed to have any underlying biological or somatic processes. Reactive depression is associated with environmental conditions.) As the industry grew complacent to accept these SSRI agents as safer and effective they were prescribed across the board for any blue mood. And dont even get me started on my rant against the use of stimulants for ADHD.
