I would like to point out that: 1) The TARDIS translates written and spoken words into a the travelers native language. Since most of The Doctors companions are from the UK on Earth, then they hear accents that sound familiar to them. We, as the audience, are hearing and seeing what they see. 2) Captain Jack Harkness originated in the 51st century. We dont know quite what his home colonys accent was like. Its possible they sounded American. 3) John Barrowman, who plays Captain Jack Harkness, is Scottish but was raised for many years in the USA. Hes stated repeatedly that he lost his native accent while in The States because of ridicule he received in school. Hes also stated, and its been shown on TV, that he does revert to a Scottish accent when hes around his immediate family. That being said, it is still kinda funny hahahah
