In patients who are fluid overloaded and oliguric, diuretics can be helpful in some cases. While there is no evidence that converting patients from oliguric to nonoliguric renal failure has a beneficial effect on mortality or recovery of renal function after AKI, avoiding fluid overload and inducing a net negative fluid balance correlate with better outcomes after AKI. For this reason, the use of diuretics in critically ill patients with AKI should be limited to management of volume overload and in some cases of hyperkalemia, when the patient is making urine.
