Please check us out at individual-expressions QDDPs - Coordinate the orderly delivery of services between the Residential/Direct Care Support Program and community services with which the Person We Serve may be involved. Must be willing to fluctuate schedule according to Individual’s needs. Primary Responsibilities Include: Facilitation of Services, Assessment and Communication, Health & Wellness, Documentation & Advocacy and Empowerment. Requirements: Bach degree, at least one year experience working directly with Individuals with developmental disabilities, CPR / First Aid / Abuse & Neglect / Blood Borne Pathogen Certifications, Strong desire, compassion and ability to help Individuals with intellectual and developmental disabilities, Capable of working effectively with a team, Personal vehicle which is insured and meets state safety and licensure requirements with a valid drivers license, and of course successfully pass criminal background check.
