Summary: The Referral Intake Coordinator is responsible for receiving all the referral phone calls/faxes and coordinating initial plans to meet the immediate needs of the patient and family. The Referral Intake Coordinator will also do initial assignment of evaluation visits, contact patient/family to notify of service that will be provided, facilitate Face 2 Face Process and owns follow through until completion. The Referral Intake Coordinator will also be required to educate all Kindred at Home personnel who accept referrals on the particulars of the Intake Process, the specifics associated with our clinical programs, products and offerings including our customer service expectations associated with each call received. The Referral Intake Coordinator will also be required to initiate appropriate communication with clinical, operational and sales teams based on the needs of referred patients and customers as outlined in the Intake Policy. Qualifications: Qualifications: Education High school graduate or equivalent required. RN or LPN preferred. Licenses/Certification Current state license if RN or LPN. Experience Minimum of 2 years in home health and/or hospice experience. Strong telephone and customer service experience with the ability to work with a diverse team. Previous intake/scheduling experience preferred. Expert working knowledge of Medicare covered home health care and other insurance programs, regulations and guidelines regarding home health care.
