May also provide care and case management or interventions designed to promote health ,prevent disease,and address barriers to access to healthcare. Job Duties &Responsibility *Provide a vast knowledge about the importance of family planning and its methods. *Investigate women abuse or neglect cases and take authorized protective action when necessary. *Collaborate with physician to evaluate patients’medical or physical condition and to assess client needs. *Refer Patient or family to community resourses to assist in recovery from mental or physical illness. *Advocate for patients to resolve crises *Modify treatment plans to comply with changes in patient status. *Monitor ,evaluate ,and record patient progress according to measurable goals described in treatment and care plan. *Identify environmental impediments to patient progress through interviews and review of patient records jobsinjo/jobs/social-worker/id/2453
