PATIENTS INFORMATION ON HYSTEROSCOPY ( Part 1). Hysteroscopy is a procedure in which the uterine cavity is inspected with the aid of a hysteroscope (a tube that carries optical and light channels or fibers). It allows for diagnosis inside the uterus and serves as a method for surgical intervention (operative hysteroscopy). Indications for Hysteroscopy. It is used as an important tool in the evaluation of the infertile woman; it is useful in a number of uterine conditions. • It is used in the treatment of uterine polyps (Polypectomy) • Asherman’s syndrome/ intra-uterine adhesions (Adhesiolysis) • Sub-mucous fibroids (Myomectomy) • Intrauterine septae (Metroplasty) And may be used for diagnosis of other intrauterine problems (diagnostic). What does it entail? It is an outpatient procedure which often can be done at an office session. The patient is in a lithotomy position (common position for surgical procedures and medical examinations involving the pelvis and lower abdomen). A paracervical block (an anesthetics injected into the cervix to reduce pain) with local anesthesia is given. After cervical dilation (temporarily stretching the cervix with a series of dilator of increasing diameter), the hysteroscope with its sheath is inserted transvaginally guided into the uterine cavity, the cavity insufflated and an inspection is performed. If abnormalities are found, an operative hysteroscope with a channel to allow specialized instruments to enter the cavity is used to perform the surgery. A distending medium in the form of a sterile fluid (saline or glycine) is used to expand the uterine cavity for better visualization. Biopsies of suspicious tissue can also be taken at the same time and sent for histology (microscopic examination).
