PCMH is drastically changing how we provide care. Dave Serlin, M.D. will be leading our efforts to transition to the health care of the future. “The idea is to change the model of health care from reactive and paternalistic –telling patients what’s best for them – to a collaborative and proactive model where patients take more active roles in decisions about their care and when, where and how they receive it,” says Dr. Serlin. “We are already implementing some of these ideas around the health system in bits and pieces. My primary goal will be to standardize how we deploy these tactics across sites and leverage the power of our size and scope in order to do it in a more organized and structured way.”
