Senior Transition Worker New Description -------------------------------------------------------------------------------- The primary model of support used for each of the projects in Broken Hill is transitional support intensive case management using an outreach approach. Indigenous Australians are the primary group of people these projects hope to impact upon. All staff will belong to the same team and use the same model of support but will be funded from different agencies with different reporting requirements.
