Sir/ Madam, Please find the attachment as ZIP folder containing the Affidavit, Mr Suresh Kumar Maurya husband of Pushpa Devi R/o 311/28 Bhur, Thana Prem Nagar, Bareilly today submits an Affidavit to CMO, ( as district Nodal Officer, RSBY Bareilly) Bareilly in matter regarding issues between IC HDFC & Krishna Hospital, Bareilly, the copy of the affidavit is attached in ZIP folder. The contents of the aforesaid affidavit shows that the action of the IC HDFC against Krishna Hospital Bareilly as issuance of Show cause notice was parti pris, partial, biased, prejudice, one sided, , partisan, discriminatory, unfair, ridiculous, arbitrary, autocratic, dismaying, monopolistic and heights of arrogance & authoritarism. The patient investigation visit dated 17/6/2014 to Krishna Hospital was deliberated, planned, a conspiracy with vested interest to remove the hospital at any cost from RSBY panel and to panelize and victimize & demoralize other hospitals and throttling the voice of network hospitals because Dr Sanjay Agarwal raised the issue of poor quality smart cards which is under investigation by DGLW and is in process to form an association “UP Private Hospital’s group of RSBY” to fight for the cause of RSBY hospitals and raising issues of harassment of IC / TPA in settlement of claims, rejections etc. and is very active in Google Net work hospital Group. The HDFC team purposefully visited his hospital comprising Mr. Ravi Shrivastava, Dr Ranjan Kumar, Mr Amit Razoria BAMS at 8.30 AM and check every possible thing & records in the hospital like an Income tax raid and leave hospital at 1.30 PM took full 5.00 hours. Ordinarily for such inspection only a qualified Doctor visit and carried out such inspection. In the said affidavit Mr. Suresh Kumar Maurya confirms the earlier facts given by Dr Sanjay Agarwal, that her wife Pushpa Devi twice visited hospital, • First on 12/6/2014 at 6.0 AM without smart card and she was advised to admit and asked if she brought the card same day and after authentication, the Rs.350/- would be refund and provided all services free of cost but due to relief in pain & symptoms and due to urgent some works the couple went back to home. • Second time she was visited next morning on 13/6/2014 at around 4.0 AM with smart card, immediately on arrival to hospital, immediately the card was authenticated & blocked and she was admitted & her treatment started. This time she was not charged any money and every services, operation, medicine, meal etc provided free and at discharge she was paid Rs.100/- as TA . • He further reveals that during her second visit, he did not bring the first visit emergency prescription and hence he never raised any claim for refund of Rs.350/- paid for emergency visit dated 12/6/2014.. • He has admitted in the affidavit their fault, error & further he himself blamed for the lapses and the matter which Dr Sanjay Agarwal & Krishna Hospital is facing and on above facts and in the affidavit he never shows any unpleasantness & dissatisfaction towards Krishna Hospital for any service rendered to her during the course of stay in this hospital. • He further states that he told to the team that on first visit (without Smart Card), she was charged but assurance was given to them that when the card was produced and authenticated (in the same day) the emergency fee charged would be refund (if she admitted) but they preferred to go home. • The family is not only satisfied but he has praised & appreciated the hospital,for its staff, commitment and services rendered to her and the fairness for the RSBY patient. See the para 1 & 2 of the affidavit which shows his unpleasantness & dissatisfaction to Ajay Pratima Hospital Bareilly & Dhanvantri Tomar Hospital Bareilly (both are empanelled hospital under RSBY). In AjayPratima Hospital, he was linger on for full one month and every investigations and US (twice) was carried out from outside but fact is that this hospital has its own lab & US machine. And under RSBY norms if the hospital has no investigation facility then the hospital must have arrangement with investigation centre to perform free investigation, during that one month he was never admitted inspite of multiple attaches of acute pain during that period and all bill of medicine, investigations were bear by the poor family and after lapse of one month the Surgeon Dr Manoj Gupta told them to come after 4-5 days, the surgeon was not available, he spoke utter lie that he did not perform Gall bladder surgery. In para 2 he revealed that than he went to Dhanvantri Tomer Hospital, were he paid Rs.200/- for consultation and doctor of that hospital told them that the expenses of the operation would be bear by them and when the money received from the company they would refund them. It is a matter of surprise that Ajay Pratima Hospital & Dhawantri Tomer hospital did the gross violation of the RSBY and Mr Suresh Kumar maurya gave in writing his grievances to the audit team but instead of taking action against these two hospitals, IC HDFC preferred action against Krishna Hospital. In the affidavit Mr. Suresh has expressed his anguish & dissatisfaction against these two hospitals and pleads for action against both hospitals. The above material facts are clearly establishing that the show cause notice against Krishna Hospital is biased, prejudice, arbitrary, abuse of power, monopolistic, unfair, unjustified with malafied intentions, to throttle the voice of network hospitals, and to reduce the claims by reducing the number of active hospitals and to gutted the RSBY in the region. The above facts and way IC HDFC working in Bareilly region is no more deserve to serve the RSBY any more, SNA UP & MoLE is requested to investigate this matter in details with other issues of the net work hospitals in the area because nearby districts of Bareilly no private hospitals are working for RSBY and in Bareilly upto 8 active hospitals are put on hold for long period of two or more months now only 2-3 hospitals are remained active but they too have reduced intake of RSBY cases considerably. This company is deserved exclusion from RSBY and must be de-empanelled from RSBY because this company has nothing to do with the RSBY and only target is maximum profit & least losses..
