Submission Release Form DATE: August 2, 2014 Dear Taylor Swift and/or Taylor Swift Team: I’m an attorney in Utah interested in having Taylor Swift star in a movie of a fairy tale romance story based on a real story. I am or will be submitting to you the following material (hereafter referred to as “the Material” or “material”). TITLE: “Mine and Jamie’s Love Story” or to be jointly determined. FORM OF MATERIAL: Written record of mine and Jamie’s love Story as found at mineandjamieslovestory.blogspot. PRINCIPAL CHARACTERS: Jamie Diane Gibson Hartley (to be played by Taylor Swift) and Taylor Charles Hartley (to be determined). BRIEF SUMMARY OF PLOT: Follow Jamie Diane Gibson Hartley’s journey of falling in love with her eventual husband, Taylor Charles Hartley, and continue to experience her 13-year life with Taylor: the best fairy tale romance story that really happened. WRITERS GUILD OF AMERICA REGISTRATION #: None. I am submitting to you with this Submission Release Form certain written material (the “Material”) identified as most currently found on mineandjamieslovestory.blogspot. All the posts associated directly with the blog on the blogger are considered part of the Material as well as any referenced links. You may also receive a hardbound, independently produced book that has less than the most current information. I provide it for convenience only. The book is titled, “Mine and Jamie’s Love Story,” with thirty-one chapters and an appendix. I recognize that your files are replete with ideas and stories for theatrical motion pictures and television programs, and that new ideas for motion pictures and television programs are constantly being submitted to you or being developed by you. I also recognize that many stories and ideas are similar and often different stories and ideas relate to one or more common underlying themes. In consideration of the foregoing and your agreeing to accept and review the Material, I agree to the following: I acknowledge that the Material was created and written by me without any suggestion or request from you that I write or create the Material. You agree that you will not use the Material or any part thereof unless you either: A. RIGHTS 1. Enter into an agreement with me granting you the right to use the Material or determine in good faith that you have the independent legal right without my consent to use all or any part of (or any features or elements in) the Material either because: (a) I do not own or control such Material or such features or elements; or (b) the Material or features or elements used by you and claimed by me to be the Material or embodied in the Material is in the public domain and not copyrighted, is not new or novel, is not legally protected or protectable, or was independently developed by you or obtained by you from other sources including your own employees. B. OBLIGATIONS I, hereby, acknowledge and agree as follows: 1. I warrant that I am the sole owner and author of the above described material and that I have the full right and authorization to submit the material to you; 2. I agree that any part of the submitted material that is not novel or original and not legally Protected may be used by you without any liability on your part to me and that nothing herein shall place you in any different position with respect to such non-novel or non-original material by reason hereof; 3. You shall not be under any obligation to me with respect to the submitted material except as may later be set forth in a fully executed written agreement between us; I realize that you may have had access to and/or may have independently created or have had created ideas, themes, formats and/or other materials which may be similar to the theme, plot, idea, format, or other element of the material now being submitted by me; and I agree that I will not be entitled to any compensation by reason of the use by you of such similar material. I have retained a copy of the Material and agree that you shall not be obligated to return any or all of the Material to me, and I release you from all liability if the Material is lost, misplaced, stolen, destroyed, or otherwise rendered inaccessible. I acknowledge that you are under no obligation to use the Material in any manner. I further represent and warrant that the material is original with me, that I am the author and sole and exclusive owner of the Material and of all rights in and to the Material, and that I have full power and authority to submit the Material to you on the foregoing terms and conditions, each and all of which shall be binding on me, my agents, heirs, successors, licensees, and assigns.
