Whats New @ JGH? NATIONAL HEALTH INFORMATION WEEK! The American Health Information Management Association (AHIMA) will honor and celebrate health information professionals during the 25th annual Health Information Professionals Week March 16‒22, 2014. This year’s theme is “Transforming Healthcare with Information.” Health Information plays a key role in the effective management of health data and medical records needed to deliver quality health care to the public. The work Health Information does to ensure the integrity of health information is imperative to clinical and administrative decision making. Access to accurate information helps everyone make important decisions and leads to a healthy society. The Health Information Department at Jackson General Hospital (JGH) want to inform patients that they have the right to inspect, review, and receive a copy of their medical records. There are three ways to request JGH medical records: in person, by fax, or by mail. To request medical records in person, print and complete the Authorization for Disclosure of Protected Health Information form and return the form to the Health Information department (medical records) release of information office, located on the second floor of the Jackson General Hospital, Monday through Friday, 8 a.m. to 5:00 p.m. A valid photo ID must be presented when picking up records. To request medical records by fax or mail, print the Authorization for Request of Protected Health Information Form. Fax the completed form to the Health Information department at (304) 372-2574, or mail the completed form to Jackson General Hospital, Attn: Health Information (medical records), PO Box 720, Ripley WV 25271. Be sure to supply the address where the records are to be mailed or the secure fax number. Records will be mailed to you within 5-10 business days to allow time for processing.
