13.3.2014 A 46-year-old magazine columnist is referred by her general practitioner (GP) complaining of tiredness and lethargy. There has been no weight loss. The GP found her serum sodium to be 128 mmol/l (137-144) and 130 mmol/l on two occasions (with a normal creatinine and normal glucose concentrations), and ordered a chest X-ray. This was reported as showing a small area of increased radiodensity in the right lung apex. Paired plasma and urine osmolalities are 255 mosmol/kg (285-295) and 148 mosmol/kg (50-1200) respectively.
