4Children and Take a Break magazine are looking for 12 heroes from across the UK who are inspirational, outstanding and have gone above and beyond to help a child, young person or family in their community. A hero can be any age, any profession, a family member or friend, as long as they have made a real and lasting impact. Nominations must be received by 31 August 2013. Each hero will receive an all expenses paid trip to London to attend the Family Heroes Awards Ceremony on Wednesday 23rd October and will have their story featured in Take a Break.
