A relative weakness of the smaller muscles involved in stabilization and fixation of the optimal lifting posture during big compound lifts can prevent you from doing multiple reps with heavy weights. If those are weak and as a result by rep 2 or 3 with limit weights they arent strong enough to allow you to maintain an optimal lifting position throughout, the prime movers will not be able to do their work properly, having to work a lot harder to do the job (thus gassing out faster). When this is the case there will often be a pretty significant difference between the weight you can lift for a maximum single and a maximum triple. You should be able to do 90-92% of your 1RM for 3 reps. If you cant do that in good form it is possible that you are suffering from the above problem. It can also show itself via the incapacity to sustain performance with near-maximal weights for several sets, despite allowing proper rest periods.
