Ankylosing Spondylitis AS - is an seronegative inflammatory disorder, that affects primarily the axial skeleton and the peripheral joints. - etiology is an know. - males affected 3-4 times more than females, by the second to third decade. - 90% of pateants with AS are +ve for HLA B-27. - it present with chonic lower bask pain in a young man, with morning stiffnes lasting at least 1h and improves by exercise. - the cervical spine will be involved rerely and ONLY at the late stage. - Extraarticular manifestations: 1.anterior uveitis. 2.aortic insufficiency - may cause congestive heart failur. 3.3ed - degree heart block. - on examination: * +ve Shober test. * obliteration of the lumber lordosis - may cause spine fractures. - X-Ray: * sacroiliitis - the earliest finding. * eventual fusing of the sacroiliac joint. * bamboo spine. * squaring of the vertebral bodies. - Treatment: * NSAID. * Physical therapy. * exercise. * anti-TNF-alpha * methatrexate. @MedKey
