Home Emergency Book The basic service includes: ~Face sheet with demographics ~Medical/surgical history ~Physical limitations ~Physicians/health care providers ~Medications ~Emergency contact/advance directives. Also contained within this packet would be 10 "face sheets" for emergency personnel to take with them containing a brief summary. The Enhanced Emergency Book contains all of the above. Included are updates every quarter at no additional charge. If major medical changes happen within that time frame, modest fee will be charged. Also- Personal Medical Record Organization. This would either be for the client, their family member, or their entire family. This would be extremely useful for the client if they have a special needs family member, especially if there is litigation involved. This is an hourly fee based service where we would go in, organize the ENTIRE medical records. This includes but not limited to: physicals, procedures, physicians, even their medical bills-broken down to their needs. A ledger could be provided to help the client keep track of balances and payments. At this time, I would like to thank Miranda Colligan for coming up with these ideas.
