So, if you do not have Medicaid (or any insurance), food stamps, or WIC, please be sure to bring the following information to club on Monday so we can help you enroll in programs that will save you tons of money: 1. Address 2. Social Security Number 3. Childs social security Number 4. Household income ( if you are living with your parents and you are under 21, you will have to ask them for their total household income) 5. Employee Information or School Information (pay stub or student schedule) 6. Daycare Information
