E C Tyree Health & Dental Clinic Case Manager Job Description Summary: Assisting patients in the coordination of social service and referral needs in collaboration with the integration care team. Responsibilities: • Interviewing clinic patients after they have seen the PCP to determine what type of assistance they need. • Determine eligibility and enroll in Prescription Patient Assistance Programs • Become knowledgeable of any other programs in the county or state so that appropriate referrals may be made. • Help patients access other services after determining their needs. • Schedule patient referrals to other physicians for appointments or to the hospital for tests. • Refer diabetic patients to the diabetic classes. • Work with agencies and service providers to facilitate case coordination and information sharing. • Maintain case records and provide assistance with appropriate documentation. • Work closely with law enforcement agencies, schools, health, and welfare agencies. • Maintain a caseload of patients. • Other duties as assigned Requirements: • Good documentation skills • Excellent written and verbal communication skills, and ability to establish rapport. • Ability to motivate others towards achieving goals. • Ability to work independently with strong sense of focus, task-oriented, non-judgmental, flexible, and able to maintain appropriate boundaries. • Clear understanding of and respect for confidentiality involving both patients and fellow employees. • Ability to legally operate a motor vehicle and provide own transportation. • Preferred Qualifications: LMSW, LBSW, LSCSW Schaunta James-Boyd Chief Executive Officer E.C.
