Forward Head Posture vs “Text Neck” Is there a difference? What is the difference? The older, more familiar term, forward head posture is characterized by the ear forwardly positioned in front of the shoulder, (see first picture). In general the more the forward position the more the head will way in relationship to the lower discs in the neck. A good guide is if the average head weighs about 10-12 lbs. then with a forward head posture the head now puts about 3-4 times the stress on the lower cervical (neck) discs increasing the chance for disc herniations especial of the C6-7 and C5-6 levels respectfully as the lower cervical spine moves into a flexed posture. Another feature of this posture is that as the lower cervical spine moves more into a flexed posture the upper cervical spine hyperextends to keep the gaze forward instead of downward. Unfortunately this leads to compression of the C2 nerve which in turn results in a typical headache in the occipital-frontal (back of the head to the forehead) region and sometimes behind the eyes, See second picture ).
