If you have received any Childcare Allowance payments from us between 1 Sept and 30 Nov 2014 we must receive your completed Childcare reconciliation form(s), your childcare receipts and your student coversheet by 31 December 2014 or you will not receive a payment on Friday 16 Jan 2015. The address to send them to is on the Childcare reconciliation form. To ensure your receipts and form reach us on time, we recommend you use next day Special Delivery post during this busy period. You can print the Childcare reconciliation form on our website at nhsbsa.nhs.uk/Students/3949.aspx. Please visit our ‘Ask Us’ page for more information about the childcare reconciliation process. If we do not receive your childcare receipts and completed, signed Childcare reconciliation form by 31 December, we will suspend your Childcare Allowance payments from Friday 16 January 2015 and seek to reclaim all unreconciled Childcare Allowance that you have been paid.
