Job Posting: Community Services Program Seniors Outreach Worker (full-time) Qualifications: A Licensed Social Worker with a B.S.W. or B.A. in a related field preferred. A minimum of three years experience in working with seniors, senior program development, or providing case management services can be substituted for licensure and academic requirements. Must be over 18 years of age with a valid driver’s license and two years driving experience. Salary range $25,000- $27,000. General Statement of Duties: The Seniors Outreach Worker is responsible for providing social services and support to seniors and disabled residents in Gladden’s service area (all of zip codes 43222 and 43223, and the part of 43215 west of the Scioto River). The Outreach Worker provides support services to community seniors in their homes and at our agency, provides group transportation for grocery trips and some social outings, and attends community meetings related to senior issues and program development. The Outreach Worker is directly responsible to the Vice President of Community Services, and is ultimately responsible to the agency President/CEO. Activities to be performed, but not limited to supervising and coordinating all agency senior services and special activities, including: • Visiting homebound seniors and disabled residents for intake and assessment and as needed throughout program enrollment (annual goal is to link with 75 seniors annually) • Maintaining regular phone contact with all program clients to monitor their status. • Transporting seniors occasionally as needed (doctor’s appointments, prescription pickups, grocery shopping, etc.) • Referring and linking clients to other agency and extra-agency services as needed, including home repairs and emergency food, housing, and financial assistance. The Outreach Worker will be trained to enroll clients as needed in emergency assistance programs like EFSP rental assistance, Neighbor 2 Neighbor utility assistance, Senior Options services, etc. • Recruiting, training, and supervising a pool of volunteers to visit and support homebound seniors and disabled residents. • Soliciting resources, contributions, and in-kind services to address client needs identified through visits or client contacts, including grant writing and presentations to potential donors/supporters. • Planning and implementing periodic special events. • Keeping accurate and up to date documentation on all client services, including weekly case notes, quarterly and annual reports, and monthly senior transportation data entry. • Providing training and shadowing experiences for OSU and Capital students. As part of the Community Services team, the Caseworker is expected to be flexible and cooperative with other team members in meeting our clients’ needs, and will be expected to assist with general department services. The Caseworker will also perform other general duties and services as directed by the Vice President of Community Services or the agency President/CEO. All agency staff are expected to develop and maintain a thorough knowledge of agency policies and procedures and of agency and community resources.
