Job Posting: ECHO Endo: Community Health Worker job description: ECFH is seeking an experienced Community Health Worker to provide comprehensive support services to patients with complex Type 1 and Type 2 Diabetes. This position includes patient care in a primary care setting and home visits when applicable. This individual will function on a team with a provider and will be responsible for patient education in the areas of blood sugar, blood pressure, diet, physical activity, medication adherence, and support systems. Strongly preferred: bilingual in English/Spanish and have experience with navigation of local medical systems and community resources. Education: High school diploma or equivalent. Experience: Prior experience, with a minimum of 1 year, in this type of role.
