Longwood Schools Expands Student Registration Hours; Schools Begin Wednesday, September 3rd Have you recently moved to the Longwood community? Register your children now for the start of school on Wed., Sept. 3. Longwood School District’s Central Registration Office for all children in kindergarten through 12th grade is open Mon. through Thurs. from 7 AM to 3PM, through Aug. 14; and Mon. through Fri., 8 AM to 2:30, from Aug. 18 through Sept. 5. The registration office is open on Tues. and Thurs. evenings until 7 PM through Sept. 18. Plan to arrive at least one hour prior to closing. The office is closed on Sept. 1 for Labor Day. Longwood’s Central Registration Office is located in the Charles E. Walters Elementary School Primary Center in Yaphank. The school is located at 15 Everett Drive, Yaphank, directly behind the Yaphank firehouse on Main Street. The office can be reached by entering the main gate of the school campus, traveling through the bus parking lot to the second building (Primary Center). Parking is available behind the school where a sign indicates Student Registration. For all students registering for school, the district requires: • Proof of residency. One of the following documents must be presented: a Town of Brookhaven tax bill, a deed, a lease with two bills, an Affidavit of Landlord (provided by the district) or custody papers. • An original birth certificate with a raised seal, or a passport. • Immunization dates for DTaP, polio, measles, German measles (rubella), mumps, hepatitis B and varicella. • A physical within 12 months prior to registration. Physical and History forms are available on the district’s website under parent information: longwood.k12.ny.us. • A transfer card or discharge notice from previous district (except kindergarten). • An IEP for students with disabilities. Uninsured parents interested in free immunizations for their children may call the Children’s Immunization Program of Suffolk County at 854-0222. Just Kids is also offering free physicals for children entering kindergarten, and free immunizations. Contact them at 924-1000 Registration of a foster child also requires that a guardian or agency present evidence or have in their possession a court order assignment and/or a Department of Social Services assignment or agency assignment that they have been given authorization to act on behalf of the State of New York. Once registered, new secondary school students must make an appointment with their guidance counselor to schedule their classes. It is suggested that secondary students bring their report cards or a copy of their transcript with them in order to facilitate scheduling of appropriate classes. Parents are asked to present a dental health certificate for children registering for pre-kindergarten, kindergarten or first grade.
