Describe how the population was identified, engagement strategies, what types of health assessment and risk stratification were used, the type of intervention based on the care continuum of low to high risk and type of PHM intervention (e.g., disease management, health promotion/wellness). Describe the intervention components and outcome measurements, and of course the results. You should focus on a synthesis of the research rather than simply re-stating findings. That means you should look for common findings across studies, areas of inconsistency, and relative strengths and merits of different studies. Look for review or meta-analysis papers on your topics as a starting point. Then select the most representative and recent research articles for inclusion in your literature review." Scuse me- huh?
