NOV 1ST IS THE ABSOLUTE LAST DAY TO NOMINATE FOR WARRIORS CHRISTMAS! Here are the criteria! Check out the page at facebook/aagfwarriorschristmas **PLEASE NOTE THIS PROGRAM IS FOR OUR LONG TERM PATIENTS EXPLICITLY! WE HAVE SPECIAL CAREPACKAGES FOR OUR NEW BABIES.** -All nominees *must* be between the ages of 2 years and 18 years old as of December 31st 2013 (No exceptions.) -They must be long term gastroschisis survivors who are medically needy and who have received long term intravenous care and have been hospitalized in the past year for medical intervention. (No exceptions) -Prior years winners and nominees may not reapply. (No exceptions) -Submissions must include a story no more than 800 words including as many major operations and medical interventions as possible along with some details of the child/teens life and no more than 10 pictures from birth to current. (No exceptions) -Nominees must be willing to have their stories and pictures shared online and their medical records verified. (No exceptions) Nominees will be announced as they are approved. 5 Warriors will be voted on and announced November 1st by the executive team of Averys Angels Gastroschisis Foundation. Special gifts will be given to all nominees. Warriors must have Christmas lists compiled and sent into to [email protected] by November 3rd. Lists will be published as they are submitted for supporters and fans to donate items off the lists or entire lists. All gifts must be mailed into AAGF by December 1st in-order to ensure on-time delivery. (No exceptions) We Love this program and cant wait for this years nominees to inspire and motivate the world!
