Nassau County Medical Center NURSE MANAGER Westbury, NY With a strong commitment to raising the bar for healthcare in our community, Nassau University Medical Center is a Level I Trauma Center and a 530-bed teaching hospital affiliated with the North Shore/LIJ Health Care System and Stony Brook University. We currently have an exceptional opportunity for a Nurse Manager to be part of our team. This position will be located at the Westbury location of The Long Island Federally Qualified Health Center, Inc, which serves about 19,000 patients annually, with 7 providers (2 family practitioners, 2 OB/GYN, 2 Medical, 1 Podiatry) Family Practice residents and a staff of RNs, LPNs, MAs, Clerks, Patient Navigators, Social Workers and a Nutritionist. In this role, you will be responsible for overall assessment, planning, implementation and evaluation of patient care at the health center; creating a patient focused caring environment while addressing and meeting the needs of the patient; and fostering an environment of collaboration and respect with the administration team, physician colleagues, nurses and the healthcare team. You will also function in the role of Care Manager. The Care Manager is responsible for the coordination of patient care, education and patient advocacy at the Health Center as well as overseeing/coordinating the care of high risk and chronically ill patient populations and making care plans that improve health outcomes for patients. We require: •Bachelor’s degree •Licensed as a NYS Registered Professional Nurse •At least 3 years of relevant experience in healthcare 1 yr supervisory experience. •BCLS Certification •Excellent communication skills, judgment, listening and interpersonal skills •Cultural and linguistic sensitivity to populations served. •Knowledge of EMR systems and computer programs (i.e. MS Office) •Bilingual in English and Spanish or Creole preferred.
