Now MONEY that should be weekly paid to you is DELAYED and you have NOTHING sound FAMILIAR??? There are Steps that must be taken when you are hurt on the job!! (1) You should consult with an ATTORNEY. (2) You should know and understand about WORKMENS COMPENSATION LAWS. (3) You should know and understand the PROCEDURES and MOTIONS your LAWYER must make on your BEHALF!! (4) Your Lawyer should tell you all of the PROCEDURES and MOTIONS he plans to FILE IN YOUR CASE!! Here are some of many things to know: (A) If there is a loss of more than 3 days of SCHEDULED WORK ILLINOIS FORM 45 [EMPLOYERS FIRST REPORT OF INJURY] Must be filed by law!! You can request a copy of this form from THE WORKERS COMPENSATION COMMISSION 4500 S. SIXTH ST. FRONTAGE RD SPRINGFIELD IL. 62703 There are BOXES to be filled out by your EMPLOYER; one of them ask what was the doing when the ACCIDENT OCCURRED?? This form can help your LAWYER in REPRESENTING you!! (B) Your Lawyer MUST FILE a 19 (b) PETITION FOR IMMEDIATE HEARING; this will speed up the process to get your MONEY!! (B) Records of your personnel file can hurt you if you have a HISTORY for DOCUMENTED INJURY!!
