Procedures for Local OJT 1. Attend the On-the-Job Training/ Related Experience/ Practicum/ Internship Training Orientation Seminar scheduled by the Office of On-the-Job Training. 2. Secure an endorsement letter/ recommendation letter (may be issued one at a time). The letter must be signed by the respective OJT Coordinator and recommended for approval by the Dean of Colleges with attached medical certificate and certificate of good moral character. (for students who are applying for OJT in companies/ industries/ schools/ hospitals). Note: A new endorsement will be released if the previous company endorsed to has not been able to accommodate the student. Students may seek the help of the OJT Coordinators to follow-up the OJT application. 3. Submit the Endorsement Letter / Recommendation Letter to the company. 4. Follow-up the update and acceptance of the application to the possible host company. Once accepted, report to the respective OJT Coordinator. 5. Complete all the requirements to be submitted to the OJT Coordinator. Documents to be submitted to OJT Coordinator before training are: - Received Copy of Endorsement/Recommendation Letter - Copy of OJT Time Frame (Certified by the Company) - Training Agreement and Liability waiver (Notarized) - Medical Certificate - Student Trainees’ Personal History Statement or Resume - Certificate of Good Moral Character - Photocopy of Enrollment / Registration Form - Location Map Form - Photocopy of Insurance Certificate - Businiess Permit of the Dormitory The OJT Coordinator upon receiving the complete documents of the student-trainee, shall then issue a Related Learning Experience (RLE) Journal and Student Trainees’ Feedback Form to the student trainee. 6. Start the On-the-Job Training/ Related Learning Experience/ Practicum/ Internship Training with the host companies/ industries schools first enrolling in the subject and submitting the above documents. 7. Submit the following documents upon completion of the On-the-Job training/ Related Learning experience/ Practicum/ Internship training program to the respective OJT Coordinators: - Daily Accomplished Related Learning Experience (RLE) - Original and Photocopy of Certification of Completion of Training from the Company. - Duly Accomplished Student Trainees’ Performance - Appraisal Report and Copy of Narrative Report Note: The grading system shall consist of the evaluation of the Host Training Establishment/ Placement/ Company and the evaluation of the OJT Coordinator.
