RN Educator- email resume to [email protected] *Must be willing to train/educate 3+ days in office Part-Time to to Full Time The Internal Nurse Case Manager will be responsible for developing plans of care in coordination with patients’ health care providers, implementation of those plans, and monitoring and evaluation of an assigned patient case load. ESSENTIAL DUTIES & RESPONSIBILITIES -Develop training for HHA/CNA and LPN on different company objectives -Clinically interview HHA and LPN for positions • Evaluates and monitors staff competencies as they relate to current clinical needs of each patient • Actively involved in internal department procedures and internal quality management program (clinical and operational quality). • Other job duties as assigned QUALIFICATIONS • Bachelors and/or Associates in Nursing from an accredited nursing school • Current license from the State of Michigan to practice as a Registered Nurse • Minimum of 3 years’ experience in hospital or other clinical setting . • Minimum of 2 years’ experience in management • Strong organizational and time management skills • Strong attention to detail, ability to anticipate patients’ needs, follow-through and ownership on assignments • Excellent verbal and written skills • Ability to deliver professional customer service to both internal and external staff and clients • Proficient in Microsoft Office • Home care experience preferred
