The applicant shall submit to the head of secondary school a written application supported by the following documents: 1. CSC Form 2012 (Revised 2005) in two copies with the latest 2x2 ID picture 2. Certified photocopy of ratings obtained in the Licensure Examination for Teachers/Professional Board Examination for Teachers 3. Certified copies of transcript of records for baccalaureate course 4. Copies of service record, performance rating and school clearance for those with teaching experience 5. Certificate of specialized training, if any 6. Certified copy of the Voters ID 7. NBI Clearance 8.
