B) Left upper lobe. Clinical Data: Worsening dyspnea with progressive interstitial lung disease. Patient history notable for scleroderma. Over the past 2-3 years she has worsening shortness of breath. Treated with steroids and other rheumatologic medications but her dyspnea has progressed. Diagnosis: A. Lung, left lower lobe, wedge biopsy: Non-specific interstitial pneum~, f~rosing patt~ (see comment) . ~ B. Lung, left upper lobe, wedge biopsy: Non-specific interstitial pneumonia, fibrosing pattern (see comment) . Comment: Both wedge biopsies show relatively uniform involvement of lung by fibrous expansion of the lung interstitia. The process appears relatively homogeneous without prominent fibroblastic foci and thus a nonspecific interstitial pneumonia, fibrosing pattern, is favored.
