For an operation of this magnitude there should have been PTW (Permit to Work system), HIRAC, (Hazard Identification Risk Assessment and Control) must be carried our SWM (Safe Work Methods) must have been formulated, Competent Supervisors, Safety Professionals and Engineers must be present.as task i carried out ..This just shows the lack of respect for human life by this Construction Contractor..3 workers have been killed by this negligence ..in fact in my opinion MURDERED..due to a lack of responsible care and due diligence MRT SITE DISASTER: Engineers had gone MIA MRT SITE DISASTER: Engineers had gone MIA PETALING JAYA: An initial investigation by the Department of Occupational Safety and Health (DOSH) has revealed that there were no engineers at the site during the installation of the parapet wall of the Sungai Buloh-Kajang Mass Rapid Transit (MRT) which resulted in a 650-tonne span dislodging and falling to the ground, killing three foreign workers. MRT Corp Sdn Bhd (MRT Corp) director of strategic communications and public affairs, Amir Mahmood Razak, said the company was conducting internal investigations to ascertain who authorised the construction workers to carry out the installation work when there were no qualified engineers supervising the work and also why the engineers were not at the site. We are also investigating who is responsible for the incident, he told theSun. Meanwhile, DOSH has also deployed its officers to the site to investigate and submit a report of their findings. Amir said MRT Corp will also deploy its team to check if its appointed main and sub-contractors are strictly adhering to the safety standard operating procedures (SOPs) throughout the 51km line. We want to make sure the SOPs are put in place till the project is completed. We have informed our project delivery partner, MMC-Gamuda, to enforce the SOPs at all times, Amir said when contacted today. He said that a temporary stop-work order at the construction site had already been issued by DOSH, but work will go on in other construction areas. In the 8.30pm incident on Monday, the 650-tonne span being laid for an overhead rail passage of the Sungai Buloh-Kajang MRT line collapsed and crushed to death Bangladeshi nationals Mohamad Faruk Khan, 38, Mohammad Alauddin Mollik, 34, and Mohammad Elahi Hossain, 27. The bodies of the three victims will be flown back to their country tomorrow once permission is obtained from the Bangladesh High Commission. MRT Corp and Syarikat Muhibbah Perniagaan dan Pembinaan Sdn Bhd, the contractor for Package VI where the tragic incident happened, are liaising with the High Commission to make arrangements for the repatriation of the remains. The next-of kin and family members of the victims had been contacted and would be waiting for the arrival of the bodies, added Amir. MRT Corp held a tahlil for the three deceased yesterday night and would perform the prayers without the bodies tomorrow at the MRT construction workers headquarters, near the site of the tragedy. Meanwhile, National Institute of Occupational Safety and Health (Niosh) chairman, Tan Sri Lee Lam Thye, said work place safety and health must be constantly monitored, especially in potentially dangerous worksites. There must be constant monitoring and supervision at MRT worksites to ensure the sub-contractors are carrying out their work in accordance with SOPs, Lee told theSun.
