THIS HAS BEEN TAKEN FROM ANOTHER SITE Safety Moment: Failure to follow procedures A 16-year old girl was struck and killed by a train that she had left 30 secs earlier. The guard had dispatched the train whilst the girl was leaning against it. The investigation concluded that it was possible that a) He expected her to move away before the train moved b) He looked briefly in her direction but did not see her (a phenomenon known as “looked but failed to see” in common, repetitive tasks) c) His attention was focused on the control panel and a large group of people on the platform. Learnings We all carry out a number of routine and repetitive tasks in our day-to-day work. We need to ensure that we give the task our full attention each time that we are carrying it out. If there is a danger that you may be distracted, ask for help, or if safe to do so, stop the task until you can give it your full attenation. The guard in this incident had a procedure to follow, and knew the procedure. His failure to follow the procedure, possibly a lapse or mistake, was life-changing. We have procedures in place, and these must be followed.
