On July 19, 1989, United Airlines Flight 232, after suffering catastrophic failure of its tail-mounted engine (resulting in loss of all flight controls), was forced to make a crash-landing in Sioux City, Iowa. Of the 296 people on board, 185 survived, and 111 lost their lives. Despite the deaths, the accident is considered a prime example of successful crew resource management due to both the manner in which the flight crew handled the emergency, and the high number of survivors (considering that the airplane was landed with virtually no control). Under the circumstances, it would not have been unusual for all aboard to have perished. As a result of their heroic actions, the flight crew became well known, in particular the captain, Alfred C. Haynes, and a DC-10 instructor on board who offered his assistance, Dennis E. Fitch.
