A 10 year old girl referred to us with her stools passing out through the vagina also...due to transrectal trauma sustained accidentally during play 2 days back. Anterior rectal wall had a large rent in it..about 2cm×2cm perforating the posterior wall of vagina as well...Hymen was intact and no signs of external injury. She was sitting over this set of wheels...playing ..when the upper wheel gave way under her weight and the connecting steel rod (axle) thrust into the rectum and across the posterior wall of vagina. No intraperitoneal injury...bladder also injured but not leaking. Catheterised and faecal diversion..a divided sigmoid colostomy done..along with thorough antegrade saline lavage to wash out the faecal matter. Hopefully her injury will heal in a few weeks time when we can close her colostomy. Be careful with children...
